$139 million in pandemic relief went unreported.

The U.S. Treasury sought to recoup pandemic relief from roughly 1,000 local governments that had not reported how they used the money. Most failures are more ordinary: a deadline sits in an inbox, a required form is incomplete, or an agency cannot see which governments are still outstanding. By the time a gap is visible, the money, grant, or public trust is already at risk.
